Barotrauma: Getting Deep-Caught Fish Back Down Alive
A fish can look lively at the side of the boat and still be unable to get home. That is the part most anglers miss. Barotrauma is not a rare offshore curiosity. It shows up on snapper trips, West Coast rockfish days, deep lake-trout water, and midwestern reservoirs when walleye, bass, and stripers slide under the thermocline in July. If you fish deeper than a tall tree is tall, you need a plan before the first bait goes down.

What Barotrauma Actually Is
Most bony fish carry a swim bladder — a gas-filled sac used to hold station in the water column without swimming the whole time. At depth, water pressure squeezes that sac. Bring the fish up and the pressure falls. The gas expands. Every thirty-three feet of saltwater is roughly another atmosphere of pressure, so a fish hooked at ninety-nine feet leaves a world that was pressing on it four times as hard as the air at your gunwale. The bladder that was a compact balloon at the bottom wants to be four times larger at the surface, and the fish cannot dump that gas as fast as the reel brings it up.
Two body types matter here. Closed-bladder fish (physoclistous) have no pipe from the swim bladder to the throat. They add and remove gas through the blood, which is slow. Snapper, grouper, rockfish, sea bass, most reef fish, walleye, and largemouth and smallmouth bass belong in this group, and they take the worst of it. Open-bladder fish (physostomous) have a duct they can use to belch gas. Trout, salmon, catfish, pike, pickerel, and many herring-like fish are built this way. They are not immune. A long fight from deep water, or a bladder that does not vent in time, still leaves them with expanded gas, damaged tissue, and sometimes bubbles in the eyes and blood. Lake trout pulled from deep summer or winter water are a common freshwater example.
The pink or white mass coming out of the mouth is almost always the stomach, shoved forward by the expanding bladder. It is not the swim bladder itself “popping out.” That distinction matters. Puncturing the everted stomach does not release the gas that is trapping the fish at the surface. It adds a wound. The rest of the fish suffers from the same pressure drop: eyes pushed outward, a belly tight and drum-like, gas under the skin or between the scales, intestines showing at the vent, internal bleeding and organ displacement you will never see, and a fish that can still flap but cannot drive itself down. Barotrauma is that whole package, not only the cartoon stomach hanging from the lips.
Why It Matters on a Real Trip
A fish that cannot get down does not get a second chance. It floats. Birds find it. Dolphins, sharks, and jacks find it. Surface water is often much warmer than the depth it came from, which adds heat stress on top of the internal injuries. Even a fish that finally sinks may have damaged eyes, a torn bladder, or organs that no longer sit where they should. How bad the death rate is depends on species, depth, water temperature, fight time, and what you do in the next minute. Untreated deep rockfish and some snapper and grouper can see very high discard mortality — in the worst combinations, most of the fish released at the surface do not make it. That is not a universal number for every fish in every lake. It is high enough, in the fisheries that matter, that seasons, bag limits, and access to deep water are now built around whether anglers send fish back down.
Which Fish, and How Deep
There is no single magic number. Thirty feet is a useful warning line, not a guarantee. Some species show trouble in the thirties. Others look normal until sixty or seventy feet and then come up looking like they swallowed a softball. Past a hundred feet, assume you will need a device unless you know that species well. Hogfish can show it in relatively shallow water. A big red snapper from seventy feet may look worse than a small one from the same depth. Deep-drop tilefish and snowy grouper are extreme cases.
On saltwater the high-risk list is familiar: rockfish and other West Coast groundfish; red snapper, vermilion, lane, and the rest of the snapper family; grouper of all the usual suspects — gag, red, black, scamp, and goliath when released; amberjack and other deep-holding jacks; tilefish; and sea bass, hogfish, triggerfish, and many other reef species. Freshwater is easier to underestimate. Walleye on reservoirs and large lakes sit on or under the summer thermocline — often thirty feet and deeper. Lake trout come up from deep water in the Great Lakes and western reservoirs. Largemouth and smallmouth park on deep structure in clear highland lakes. White bass, hybrid stripers, and striped bass suspend over river-channel ledges. Crappie, yellow perch, and burbot hold in deep, cold water. A midwestern walleye in July, a highland-reservoir striper over an old river channel, and a Great Lakes lake trout in eighty feet of water are the same problem in different jackets.
Open-bladder fish can vent, and some pelagics manage buoyancy differently. Sharks have no swim bladder. That does not make deep-water release automatic — exhaustion, hook damage, and warm surface water still kill fish — but it is a different injury.
What You Can Prevent Before the Fish Comes Over the Rail
You cannot repeal physics. You can stop making it worse. Know the depth before you drop. If the graph says fifty-five feet and you are fishing snapper, grouper, rockfish, or summer walleye, the descending gear should already be rigged on a spare rod or a dedicated handline. Digging for it after the fish is floating is how floaters happen. Do not yo-yo the fish. Letting a deep fish run back down and pumping it up again expands and compresses the same tissues twice. Fight it as steadily as the tackle allows and get a decision made at the boat.
Shorten the surface interval. Gas keeps expanding while the fish lies in the sun. Every extra photograph, every extra livewell minute, every extra argument about whether it is a keeper, makes the next step harder. Decide keep or release on the way up if you can. Match the fight to the job. A long, under-gunned fight from deep water dumps lactic acid into a fish that then has to survive a pressure injury. Use tackle that can land the fish without horsing it through structure, then get it back down.
Two Tools, One Preference
There are two legitimate methods: send the fish back to pressure, or let a measured amount of gas out of the bladder through a hollow needle. Sending it back down is the better default in almost every saltwater reef and rockfish fishery. Freshwater results are more mixed by species and device, but a well-weighted descent still avoids putting a hole in the fish. Which tool you must carry, and whether venting is allowed at all, is a regulation question. The biology below does not replace that check.
Descending devices
A descending device is any rig that takes the fish back down and lets it go. The extra pressure recompresses the gas. Eyes settle. The stomach often slides back. The fish can swim. Common types are lip clamps and pressure releases (a clamp on the jaw with a weight — some release at a set depth, some when you jerk the line); inverted barbless hooks (a heavy weight below an upside-down barbless hook through the soft tissue of the jaw — drop the fish, then a sharp lift pops it free); and fish elevators (a weighted crate, basket, or inverted container, useful when several fish come up at once so they can swim out when they are ready).
A working rule of thumb is about one pound of weight for every five pounds of fish. Too little weight and a big snapper kites in the current or hangs in warm water. Some federal reef-fish definitions have called for at least a sixteen-ounce weight and sixty feet of line on a device that can actually release the fish. Confirm the weight, line, and device definition that applies where you are fishing.
Send the fish as close to capture depth as you can manage. If that is clumsy, get it down at least sixty-five feet or roughly half the capture depth, whichever puts it back into cooler, higher-pressure water. Studies on red snapper and red grouper have shown a clear step-up in survival when fish were sent to about twenty meters rather than stopped shallow. Do not drop a fish into a thermocline soup of warm water and leave it there if you can reach the depth it came from. Keep the fish in the water while you clip on. Work over the side, not across a hot deck. Drop it head-down. Watch the line go slack or the release pop, then wind up. You do not need to vent and descend the same fish if the weight is enough to take it down.
Venting — only if you will do it right, and only where it is allowed
Venting means opening the swim bladder with a sharpened, hollow tool so the extra gas can leave. A knife, an ice pick, or a solid spike is not a venting tool. Those make a cut without a path for the gas and they wreck tissue. In fisheries that define the tool, a hollow needle about sixteen-gauge or larger is a common standard. Confirm the legal definition where you fish; some waters ban venting for certain species even when a needle is legal elsewhere.
If local rules allow venting and you have trained for it, work on a cool, wet surface and be quick. Lay the fish on its side and fold the pectoral fin back. The insertion point is just behind that fin, two or three inches toward the tail — not in the belly, and never in the stomach hanging from the mouth. Slide the needle under a scale at about a forty-five-degree angle, only deep enough to reach the bladder. Listen for the hiss. When the hiss stops, pull the needle and release the fish head-first.
About half the anglers who think they know how to vent still pick the wrong spot. Wrong-spot venting kills fish that might have lived. That is why so many agencies now say: descend when you can; vent only when the boat is too busy, the device is fouled, or you have trained for it — and only when the current rulebook still allows it. Some groundfish fisheries go further and advise not to vent at all. Carry a descending device and use it. Do not touch the organs coming out of the mouth. Know both methods and to read the current local advice — don't start punching holes in every traumatized fish.
Skip the needle when a descending device is ready and heavy enough, when local rules prefer or require descending, when the fish came from shallow water and can swim down on its own, when you cannot find the landmark behind the pectoral fin, or when you have never practiced on a dead fish or taken a short training course. If the fish is already bleeding hard, torn up, or cooked from a long fight in July surface water — and it is legal to keep — keep it.
What the Rules Actually Ask of You
Regulations change. Memorizing last year’s pamphlet is how people get tickets. Last year's pamphlet is not the law on your water today. Every state and federal waters have published some version of the same idea: a descending device — and in some waters a legal venting tool — onboard and ready while you are fishing, is usually requried. California’s published advice has gone further and told anglers not to vent rockfish at all. “Rigged and ready,” where that phrase is used, means the device is not in the shrink wrap under the bunk. It means a weight, a line, and a device you can clip on without a treasure hunt.
Those rules exist because dead discards count against the quota. When enough anglers send fish back down, the mortality number used in the assessments drops, and that is one of the few levers that can keep a season open or a depth open. Your device is not a courtesy. It is part of how the fishery is being held together. Always confirm the current requirement for the water you are on. On-Water Checklist
Before the first drop in deep water, confirm the current regulations for this water, this species, and this date. Rig the descent device with the weight attached and enough line for the depth you marked. Carry a venting tool only if you know the insertion point and current local rules allow it. Keep a dehooker and cutters where your hand already goes, and have a plan for a second fish coming over while the first one is going down.
When the fish is at the boat, decide if you will keep or release it fast, using today’s size, bag, and season rules. If you are releasing, clip on in the water and drop. Do not pose it in the sun for a photo first. If it still floats after a descent, you did not send it deep enough or you did not use enough weight. Try again once. Do not keep yo-yoing a spent fish.
Conclusion
Barotrauma is not only the ugly fish. It is also the “pretty” one that quits six feet down and drifts back — the walleye that looks perfect on the ice and cannot hold itself upright, the rockfish that blinks at you and then lies on the surface like a board.
